Does that mean McEntire has had plastic surgery? To answer that, board-certified plastic surgeons have weighed in over the years, offering professional insight into the singer’s seemingly ageless appearance.

1960s
In the 1960s, McEntire was just beginning the journey that would eventually establish her as one of country music’s most enduring stars. Raised on a ranch in Oklahoma, she came from a rodeo family and initially gained recognition for her singing at local events.

1970s
In the 1970s, McEntire began to make her mark in country music while still in her twenties. Her youthful appearance featured a defined nose, full cheeks, and her now-iconic red hair.
Dr. Gary Motykie, a renowned plastic surgeon, reviewed McEntire’s facial evolution and noted her natural features in her 30s during the 1990s.

Her eyebrows sat slightly high and arched, giving her a naturally expressive look, while her lips — particularly the upper one — were noticeably thin, a characteristic that remained consistent throughout her career.
1980s
By the 1980s, McEntire had firmly established herself as a rising force in country music and quickly gained momentum with a string of successful albums that showcased her traditional country sound and vocal power.

This decade marked her breakout era, with hits like “How Blue” and “Whoever’s in New England” earning critical acclaim and commercial success.
1990s
Dr. Gary Motykie, a renowned plastic surgeon, reviewed McEntire’s facial evolution and noted her natural features in her 30s during the 1990s. At this point, Dr. Motykie still saw no clear evidence of cosmetic procedures.

He highlighted her cute upturned nose, youthful brows, and contagious smile. “This was pre-Botox and fillers,” he said, emphasizing that the country star’s appearance at the time reflected a natural aging process.

In the 2000s, when McEntire was in her 40s, subtle changes in her appearance began to emerge.
“She was thin, athletic, and looked great,” he added, pointing out that there were no obvious signs of cosmetic intervention during this stage of her career.

At this point, her features, including her midface structure, expressive brows, and characteristically thin upper lip, remained consistent. “She has denied ever using Botox and fillers, so probably not using them at least at that time,” he noted, crediting her youthful appearance to natural aging and good genes.

In 1999, McEntire attended the 34th Annual Academy of Country Music Awards, and her signature features were on full display.
With her bright auburn pixie cut, defined brows, and naturally thin upper lip, she looked polished and radiant. Her luminous skin and elegant white gown completed the effortlessly glamorous red carpet look.

2000s
In the late 2000s, when McEntire was in her 50s, subtle changes in her appearance began to emerge. Dr. Motykie observed increased fullness in her midface, suggesting the possible use of fillers.

“I’m a little suspicious there that she might have been playing around with some fillers,” he said. Her lips remained largely unchanged, still reflecting her naturally thin upper lip.
During the 2010s, when McEntire was in her mid-50s to mid-60s, Dr. Motykie noted more pronounced signs of cosmetic maintenance.

He also pointed to her dedication to fitness as a major factor in maintaining her facial structure, adding, “She kept her body in shape, which also keeps her face in shape.”

These subtle changes could be seen in 2006 when McEntire attended the CMT Giants event held in her honor at the Kodak Theatre in Hollywood. At 51, her brows were well-positioned, and her midface notably full.

In 2008, McEntire was photographed alongside Alan Jackson in the audience at the CMT Awards in Nashville. At 53 years old, she appeared refreshed and youthful, with a noticeably fuller midface.
Her signature red hair was styled in soft layers with bangs, framing her face and drawing attention to her smooth skin and well-defined features.

Her thin upper lip, a consistent hallmark of her look, remained unchanged, while her bright eyes and polished complexion reflected the subtle, well-maintained enhancements she was suspected of embracing during this period.
2010s
During the 2010s, when McEntire was in her mid-50s to mid-60s, Dr. Motykie noted more pronounced signs of cosmetic maintenance. The midface fullness appeared more consistent, and he became increasingly suspicious of techniques like fat grafting or filler use.

While her overall appearance remained balanced, small details — such as the fullness in her lower lip — hinted at subtle enhancement. Still, the changes were conservative and carefully executed.
In 2013, facial plastic surgeon Dr. Travis Shaw also weighed in after the country singer’s appearance at the Grammy Awards. His take?

“It’s quite obvious that Reba has had a facelift, and most likely cheek implants or filler like Juvederm or Radiesse,” he said. He called the result “a bit on the unnatural side” but praised her overall appearance, especially her physical fitness.
Now in her late 60s during the 2020s, McEntire has continued to appear remarkably youthful.

By the time she attended the 50th annual CMA Awards in 2016, McEntire was 61 and still exuding a striking red carpet presence.

Her features appeared lifted, and her upper face, particularly around the brow and eyes, looked more open and refreshed — supporting speculation of a conservative brow lift or upper eyelid procedure.
The consistency in her overall look pointed to gradual changes rather than dramatic alterations, reinforcing the expert consensus that any work done was strategic and understated.

2020s
Now in her late 60s during the 2020s, McEntire has continued to appear remarkably youthful. However, the consistency of her look has prompted deeper analysis. “Nobody just doesn’t age,” Dr. Motykie said.
He suspected a conservative facelift, brow lift, and possible fat grafting. He also noted a change in her smile that could result from the repositioning of facial tissues during surgery.

“Sometimes after a facelift, you can see a slight change in a person’s smile,” he explained. Despite these enhancements, the singer has preserved her most recognizable features. Dr. Motykie pointed out:
“She left her nose and signature hairstyle alone. That was smart…people aren’t distracted by bigger changes, and it distracted everybody from the work she may have done.”

Facial plastic surgeon Dr. Richard Westreich also observed signs of cosmetic enhancement in McEntire’s appearance during this period. In 2023, he pointed out noticeable fullness in her face and suggested she may have undergone a combination of procedures to maintain her youthful look.

“It is Reba who has started to look puffy in her face. For Reba, I would say do not get any fuller in her face,” he remarked. He revealed that the singer’s procedures likely include a rhinoplasty, brow lift, upper eyelid lift, facelift, and fat transfer to the midface.
However, despite the speculation, McEntire has denied getting work done.
He also noted the possibility of routine use of Botox and other volume-maintaining techniques, such as fillers or grafting. “It is possible Reba had a mini nose job when she was young,” Dr. Westreich added.
According to him, the country star’s overall facial volume and smooth features suggest a consistent maintenance routine that has likely spanned more than a decade. “Her routine maintenance looks to have been done for around 15 years,” he said.
At NBC’s Opry 100: A Live Celebration in March 2025, McEntire, about to be 70, stepped onto the red carpet with a polished and youthful appearance. Her skin appeared exceptionally smooth with minimal visible wrinkling, and her facial contours remained firm.
The fullness in her midface and the lifted quality of her eyes suggested ongoing intervention, yet the effect remained subtle and cohesive.

With her signature red hair styled in soft waves and her makeup expertly balanced, McEntire presented a refined version of her iconic look. However, despite the speculation, McEntire has denied getting work done. She stated in 2009:
“I don’t do Botox. I just don’t do it. Everybody else can, it’s fine with me. I don’t. It’s botulism, so I didn’t want to put that in my body. I think plastic surgery’s great if that’s what you want to do.”
Instead, she has long emphasized the importance of a consistent skincare regimen. “I take my makeup off every night, cleanse my face,” she once explained.
She said she washes her face each morning with her preferred skincare line, applies a rich moisturizer, and never skips sunscreen. She also chooses makeup and products that allow her skin to breathe as much as possible.
McEntire added about her healthy habits, “I get facials as regularly as I can, and drink lots of water. And I do take fish oils. I wear one contact. I do mono-vision, and the fish oil and EPA helps my eyes retain their moisture so my contacts don’t get dry.”
While Reba McEntire herself has publicly denied using Botox, the consensus among plastic surgeons is clear — her look has likely been enhanced subtly and strategically over time.
The procedures may include a facelift, brow lift, fat grafting, and volume maintenance — all done with a light touch that preserved her signature features. And that, experts say, may be the real secret to why McEntire looks so good at 69.
